Commitment Contract

Enrollment Agreement & Commitment Period

All participants, or their parent/guardian, must sign this agreement prior to enrollment. This is a binding one-year (10-month) commitment with Gonzales Dance Academy.

Commitment Period:

  • Start-of-Year Enrollment:
Commitment runs August through May (10 months).

  • Mid-Year Enrollment:
Commitment runs January through December (10 months).

Regardless of the enrollment start date, all participants are required to complete a full academic-year commitment of ten (10) consecutive months.

  • Tuition & Fees:

    • Monthly tuition is due on your assigned date.

    • $35 late fee if payment is not received within 5 days.

  • Requirements:

    • All required Academy items (uniforms, costumes, apparel, accessories, etc.) must be paid in full within 2 months of enrollment.

  • Financial Responsibility: Accounts unpaid after 2 months will be sent to collections.

  • Mandatory Signature: Enrollment is not complete until this contract is signed. Returning the signed confirmation ensures your commitment is on record.

  • Purpose: Signing this contract guarantees your child can fully participate in and enjoy the complete Folklorico experience.

Policies

  • Attendance: Attend all scheduled classes. Report absences via email at least 2 hours before class.

  • Tuition & Fees: Due monthly (see Commitment Contract). $35 late fee if not paid within 5 days. Apparel, shoes, and costume fees due within 2 months.

  • Uniforms: Required uniforms and props must be worn each class. Exceptions only with Director John Gonzales approval.

  • Behavior: Zero tolerance for disrespect, disruptive behavior, or unsafe actions. Misconduct may lead to warnings, suspension, or removal. Tuition remains due even if removed.

  • Announcements & Changes: Mandatory extra practices — notify Director John Gonzales if you cannot attend, then email Jesus Gonzales. Schedule changes and cancellations communicated by email/text.

  • Commitment: Full academic year commitment: August–May (first semester) or January–May (second semester). Tuition remains due even if attendance stops.

  • Collections: Accounts unpaid after 2 months will be sent to Financial Credit Network, Inc.. All balances, including late fees, remain family responsibility.

  • Required Info: Age, city of residence, and emergency contact.

  • Policy Changes: Academy may update policies at any time. All changes communicated to families. Decisions by Academy leadership are final.

This version is clean, readable, and perfect for a website page. Parents can scan it quickly and understand all expectations without getting lost in long paragraphs.

If you want, I can also create a visually organized version with headings and icons for each policy, which looks more engaging on a website. This helps parents absorb it faster. Do you want me to do that?

Expectations

  • Stay Engaged: Parents are encouraged to attend the last 5–10 minutes of class for announcements and questions. This keeps everyone informed and connected.

  • Family Connection: Communicate with other families about practice or steps—sharing tips helps everyone grow. The Academy does not provide practice videos.

  • Optional Participation: Performances, parades, and special productions are optional but encouraged—these are opportunities for students to shine and enjoy the full experience of Folklorico.

  • Respect & Cooperation: All students and parents should be kind, supportive, and respectful toward instructors, staff, and fellow families.

  • Positive Attitude: Come ready to learn, dance, and celebrate Mexican culture. Commitment to the Academy isn’t just about rules—it’s about participating fully and enjoying the journey.

Waiver and Liability

All participants, and the parents/guardians of children, acknowledge and agree that:

  • Dance classes, rehearsals, events, and productions — at any Academy location (Visalia, Porterville, Fresno) or off-site venue — involve physical activity and carry inherent risks, including slips, falls, collisions, or other injuries.

  • Gonzales Dance Academy, its directors, instructors, and staff are not responsible for any injuries, accidents, illnesses, or medical expenses incurred during participation.

  • Parents/guardians are fully responsible for all medical care and related costs for their child.

  • The Academy is not responsible for lost, stolen, or damaged personal property.

  • Participation is voluntary, and by enrolling, you assume all risks associated with activities, including travel to and from events.

  • This waiver applies to all classes, rehearsals, performances, productions, and special events, on-site or off-site.

By signing, participants and parents/guardians release Gonzales Dance Academy, its directors, staff, and volunteers from any claims, demands, or legal action arising from participation.
